錄取資訊與接機調查 Admission Info & Airport Pick-up Service Form	無論您是否需要接機服務，都請詳細填寫您的班機及抵台時間。 Please fill out your arrival information whether you need pick-up service or not. Thank you!

Important Notice

●

Please check whether your home university has an exchange agreement with NTNU. If yes, please note that you should be nominated by your home university’s exchange coordinator.

●

Required documents:

●	Application form – Signed and attach 2 passport-size photos
●	Photocopy of applicant's passport x 1
●	Letters of recommendation x 1
●	Study plan (approx. 800 to 1000 words)
●	Official academic transcripts x 1 (for undergraduate or graduate studies).
●	Student ID or enrollment certificate x 1 *All the documents must be either in English or Chinese.

To complete the application process, please have all the application forms and required documents ready and submit to the exchange coordinator.

●

For students with Chinese citizenship, please also enclose the 大陸地區人民入出臺灣地區申請書 , two passport-sized photos and Proof of Enrollment.
